- Third put me on hold for 20 minutes before I hung up
- Fourth finally gave me gold: “Call DSHS in Austin”
When I finally got a human at Texas Department of State Health Services (DSHS), I nearly cried. Lady was actually helpful – said the magic words: “Download the Body Art Facility Application packet.” FINALLY.

Paperwork Hell
Thought the hard part was over. Nope. Opened that PDF and it might as well have been rocket science. Needed:
- Business registration documents
- Floor plans of the tattoo space with equipment layout
- Autoclave sterilization machine certification
- Bloodborne pathogen training certificates
- Inspection scheduling
- And get this – $800 non-refundable fee
My buddy saw the list and almost fainted. Almost talked him into giving up right there.
Getting It Done
Broke it down step by step over cheap coffee at his garage:
- Registered his studio as an LLC (cost another $300)
- Measured every inch of that garage and drew floor plans
- Took online OSHA bloodborne pathogens course ($25 but worth it)
- Found a used autoclave on Craigslist and paid extra for certification
- Made four copies of everything – front and back
- Sent that fat stack certified mail with tracking
Waited six weeks biting nails. Then – miracle! – got an inspection date. Inspector showed up, checked sink placements, looked at sterilization logs, tested outlets. After 45 minutes? Slapped an “APPROVED” sticker on his door.
Now my buddy’s fully legal. But honestly? If I’d known it was THIS much hassle? Would’ve just tattooed “GET PERMIT FIRST” on his forearm when we started.
